Do You Register the Specific Medical History During Every Treatment?

Medical questions regarding the patient’s medical history are mostly answered during a first consultation or treatment. But actually physicians are required to check the medical history during every consultation as this could change over time and especially before performing a treatment. For example think of use of medication, developed allergies, other current treatments etc. Due to the fact that this is time consuming, some physicians register the medical history only once or a few times but unfortunately not during every consultation or treatment.

An EMR (electronic medical record) which keeps track of the medical history assists physicians to check the current medical situation fast and efficient.
